BILLING
Will you file my insurance?
To aid our patients in their overall health care, our office will submit claims to your insurance company. As a courtesy we will verify your insurance benefits prior to your appointment, however, due to the enormous number of plans it is impossible for us to determine the exact benefits and/or coverage guidelines that each plan provides. Therefore, we must rely on you to be knowledgeable about your insurance plan and coverage benefits.
Am I required to pay anything for my visit?
Most insurance plans require patients to pay a co-payment, co-insurance and/or a deductible. These payments are due at the time of your visit. We accept cash, checks and credit cards
Do I need a referral?
This depends on your insurance. If your insurance has designated a primary care physician (PCP), you are required to have a referral prior to your office visit and procedure. If this referral is not provided you will be asked to either reschedule your appointment or pay for you visit at the time of service.
What if I do not have Insurance?
It is expected that payment in full be made for office visits for patients not covered by insurance. Procedure balances may warrant an extended payment plan. A financial consultant will be glad to assist you during your office visit in order to initiate a payment plan. Payment plans that are not paid according to the contract will incur late fees. Patients who fail to make a monthly payment will be sent to a collection agency.
What will happen if I am turned over to a collection agency?
Upon receipt of your statement, payment is due in full unless prior settlement has been arranged. If your account becomes necessary to turn over to a collection agency the fee charged by the collection agency, including any legal fees will be added to your balance. Once you account has been turned over to collections we will no longer be able to assist you with your account, and no further appointments will be afforded to you until balance has been paid in full.
MEDICAL RECORDS
How do I get my medical records sent to your office from another physician?
You will need to complete a Medical Records Release form, and submit to your physician.
How do I get my medical records from your office?
To release a copy of your medical records, we need a signed authorization form from you and the address where you would like the records to be sent. Please allow up to 14 days for processing.

MEDICATION:
How do I get a refill on my medication?
Please contact your pharmacy and ask them to send your refill request to our office electronically. In the event your pharmacy is not able to request this electronically, please contact our nurse line and leave a message indicating your name, date of birth, prescription name, strength, and how often you are taking your medication. Please expect at least 2 business days for this to be completed. You may want to confirm with the pharmacy that your prescription is available before picking up medication.
When should I request a refill?
In order to receive your refill in a timely manner and avoid any gap in your treatment, please do not wait until you run out to request a refill. Contact your pharmacy or our office when you have at least 1 week of your medication left.
Can I get a refill after hours?
No medication refills will be authorized after regular office hours. The on call physician is available for medical emergencies only.
PROCEDURES
I misplaced the prep instructions for my procedure, what should I do?

How long will it take to get the results from my procedure?
After your procedure the doctor will tell you what he or she found. If a biopsy was taken at the time of your procedure, results are generally available within 10 days.
How do I retrieve my test results?
Because we realize that receiving your test results as soon as possible is important to you, we have implemented Labcalls within our practice. This service will allow you to retrieve your test results 24 hours a day, 7 days a week. You can access this system through the Test Results page, or by telephone. Test results may be retrieved 5-10 days from the date of your test. You will also receive an automated reminder call if you have not retrieved your test results within 14 days.
Will someone ever contact me directly with my test results?
In some cases you will receive a phone call directly from our nurse with your results. If you have not received a call from our nurse, or find there are no test results in the Labcalls system after 14 days, please contact our office nurse directly.

EMERGENCIES
Who do I call if I have an emergency after hours?
If you are experiencing a true medical emergency immediately call 911 for assistance. One of our physicians is on-call 24 hours a day for urgent medical needs and advice. If an urgent condition or question arises that cannot wait until regular office hours, please call our office and our answering service will contact the on-call physician who will return your call as soon as possible. Please do not call the on call physician for non-emergencies such as prescription refills or test results.
